Award Acceptance and Consent Form

Congratulations! You have been selected to receive RecoveryPeople's Lifetime Achievement Award, which is awarded to an individual in recognition of their life’s work promoting recovery from substance use and related mental health challenges. Nominations are solicited from the Board and staff. The award recipient must:
  • Publicly self identify as a person in recovery from substance use or co-occurring issues and/or an affected family member in recovery. (Recovery is defined as a process of change through which individuals improve their health and wellness, live a self- directed life, and strive to reach their full potential);
    • AND
  • Demonstrate accomplishments that reflect RecoveryPeople's mission, vision, and values.
