This form is for Qualified Supervisors seeking to supervise 250 peer specialist experience hours and/or 250 peer specialist supervisor experience hours.

A Training Entity is an organization authorized to train individuals with lived experience of mental health, substance use, or trauma to become certified peer specialists.
These entities provide curriculum covering recovery principles, ethics, and advocacy, preparing peers to mentor others, facilitate support groups, and navigate systems.
