Course Description: This training examines the role of peer support during hospital and institutional discharge and community transition. Participants will explore why discharge is a critical reentry point and how peer specialists provide non-clinical, recovery-oriented support during early transitions. The course emphasizes role clarity, ethical boundaries, and peer-appropriate support strategies that differ from clinical care, discharge planning, and case management. Participants will learn how peers support understanding, choice, and connection during periods of increased risk, confusion, and reduced structure following discharge.
Learning Objectives
Upon completion of this course, participants will be able to:
- Describe what reentry from hospitals/institutional settings means
- Explain the peer role in transition
- Identify how peer support differs from clinical or discharge functions
- Apply peer-appropriate support strategies during reentry transition
